Medical Records and Patient Portal Access
The release of information hours for medical records at Memorial’s Health Information Department are 9 am – 3 pm on weekdays for walk-in requests. The department is located in the Memorial Medical Center Annex and can be reached at (575) 521-2297. The fax for Medical Records is 855-810-1934.
We also want to remind patients of our Patient Portal, a convenient, secure health management tool you can use anywhere you have access to the Internet. It provides access to important information about you and your visit(s) to our hospital. Online healthcare information is available to patients 18 years and older and who have been a patient at MMC on or after April 1, 2014.

- Clinical Summary Documents
- Procedures
- Immunizations
- Lab Reports
- Radiology Reports
- HIM Reports
- Allergies
- Medications
All Discharged Patients who supplied a valid email address will receive an electronic invitation from the hospital and a link to register online shortly after discharge. If you have any difficulty registering, please contact by phone 1-800-851-5043 or by email patientportalsupport@syntellis.com.
