The release of information hours for medical records at Memorial’s Health Information Department are 9 am – 4:30 pm on weekdays for walk-in requests. The department is located in the Memorial Medical Center Annex and can be reached at 575.521.2297. The fax for Medical Records is 855.810.1934.

Requests will be processed within five to ten business days of receipt. Requests are fulfilled by our medical records partner, Verisma.
