Memorial Home Health moves onto MMC Campus

Las Cruces, New Mexico - Memorial Home Health moved from its location on University Ave. to the Memorial Medical Center campus in October.

Memorial Home Health offers a specialized program for Wound Care, as the only Home Health agency in Dona Ana County with a Certified Wound Ostomy Continence Nurse on staff.  Their Congestive Heart Failure program was developed to decrease the chance of a patient being re-admitted to the hospital after a heart failure. Other programs and services offered at Memorial Home Health include Diabetes Care and Management, IV Infusion, and a Comprehensive Physical Rehabilitation Program.
Memorial Home Health is Medicare Certified and has earned the Gold Seal of approval on accreditation by The Joint Commission.  MMC’s Home Health program had no negative findings during the last two Medicare/State surveys in 2009 and 2012, and no negative findings in the last two Joint Commission surveys in 2010 and 2013.

Being on the hospital campus, our team members have a greater opportunity to be more involved with the hospital’s vision,” said Director of Memorial Home Health Sue Ann Monger. The Home Health team is made up of Skilled Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, Medical Social Workers and Home Health Aides.

Memorial Home Health’s service area includes the greater area of Las Cruces and Dona Ana County.  Services are available 24 hours a day, 7 days a week.  Most insurances are accepted and Spanish-speaking are staff available.  For more information on Home Health, please call (575) 556-6500.
