Anticoagulation Management Service
Patients taking the blood thinner Coumadin need to monitor their dosage on a regular basis and have their blood tested regularly to prevent complications that may arise, such as blood clots or bleeding.The Anti-Coagulation Team evaluates patients in just a few minutes. During each visit, patients will be assessed for new medications, dosing changes, herbal remedies, dietary factors, or lifestyle modifications that could affect test results. Under the direction of a qualified medical director, pharmacists and nurse clinicians will help the patient to manage their anticoagulation therapy safely and effectively in an outpatient setting. AMS is available through referral by a physician.
